If you have an employee who needs to test outside of your company’s standard clinic network, use this form to request a One-Time Clinic Setup. Each request will incur a $35 fee added to your monthly invoice.

A member of our Clinics TEAM will promptly begin processing your request. Standard clinic setups will be completed within 24 hours. Post-accident and reasonable suspicion setups will be completed within 2 hours.
